Our patients, team members, and their families are the center of all we do as a company. Mountain State Oral & Facial Surgery is closely monitoring guidelines from the CDC and are dedicated to keeping people safe and healthy.

We are resuming normal business hours and operations including all elective and non-elective treatments. As we re-open we want our patients to know they will see some changes in our offices including but not limited to social distancing measures, increased sanitization, increased use of PPE, and other changes. These changes are to protect our staff and patients as we are committed to serving the needs of our community during these difficult times.

Patient Information

First visit

Your first visit to our practice will consist of an oral exam and a consultation with one of our expert oral surgeons. We will examine and diagnose your condition during this comprehensive evaluation and design a treatment plan specific to your individual needs and goals. Your comfort, safety, and satisfaction are always our highest priority.

Please arrive a few minutes before your scheduled appointment time to allow time to fill out our initial forms. If you would prefer to fill them out in advance, you have the option to complete the forms online.

In order to make the most of your consultation, please bring the following items with you:

All patients under the age of 18 must be accompanied by a parent or legal guardian to all appointments.

If you need assistance gathering any medical records, X-rays, or 3D scans from your dentist or primary physician prior to your consultation, please let us know. Our staff will be happy to assist you.

During your consultation, we will discuss your diagnosis and explore treatment options to determine the best course of treatment. We welcome you to discuss any questions or concerns that you may have during your consultation.

If this is a pre-operative appointment, and you will be undergoing IV anesthesia, a consultation and physical examination is mandatory. Please do not eat or drink anything after midnight, or at least 8 hours before your surgery. You will also need to bring a designated driver to stay in our waiting area to drive you home after your procedure.

If you have diabetes, high blood pressure, or any other medical condition that may be of concern, or are currently taking any medication, please let us know prior to your surgery.

Anesthesia options

We offer a variety of anesthesia options to ensure that our patients feel comfortable and at ease during treatment. During your initial consultation, we will determine the most appropriate method based on your procedure, overall health, and individual preference. 3D Imaging

Our practice utilizes advanced technology and imaging to ensure accurate and effective treatment. 3D imaging allows us to view your condition from every angle and create a personalized treatment plan catered specifically to you.


How you care for yourself is incredibly important to the success of your treatment. While our doctors have undergone extensive training to be able to provide the highest quality care, we also rely on you to adhere to proper pre- and post-operative care. To help you do so, we will provide you with pre- and post-op instructions, which will include specific guidelines for you to follow.

These instructions are intended to help you navigate both the day before your surgery and the recovery period afterward. Anything you need to do before your procedure will be identified in your pre-operative instructions. And any steps you need to follow to heal properly will be outlined in your post-operative instructions. Our goal is to make sure you have a positive experience, comfortable recovery, and optimal results. Insurance & financial information

Your care is our top priority at Mountain State Oral & Facial Surgery. To best assist you, we offer reasonable prices for your consultation and oral surgery appointments and accept all major credit cards and payment methods. Acceptable forms of payment include:

Unless prior arrangements have been made, you are responsible for making your payments in full for every appointment. If you would like to discuss your account with a member of our staff, please contact our office at any of our locations.

On the day of your surgery, our staff may be able to help you submit a claim to your insurance. We accept most insurance providers, and you may be sent a statement of your bill once your insurance company has issued a response to your claim. Any remaining or outstanding balance on your bill that is not claimed by your insurance is your responsibility.

Monday-Thursday: 8:00 AM – 4:00 PM
Friday: 8:00 AM – 2:30 PM

We offer Saturday appointments on an intermittent basis. Please contact our office for more information. We are closed on Sundays and all major holidays.

If you are coming in for a consultation, please arrive a few minutes before your scheduled appointment time to allow time to fill out our initial forms. If you need to cancel for any reason, please notify us at least 24 hours in advance. You may reschedule your appointment with us at your convenience.

Please contact our office as soon as possible if you are experiencing an emergency, severe pain, or facial trauma. Many times, we will be able to schedule an immediate appointment. As emergency situations may arise at any time and cause unforeseen delays in scheduling, we appreciate your patience and understanding.
